Eligibility
Employees who meet the eligibility requirements based on weekly hours worked may participate in the benefits program. Benefits begin on your 30th day of employment for full-time and 75% part-time beneficiary employees. Coverage will end on the date of termination or resignation.​
If you don't enroll by the date when your benefits will go into effect, you will not have the opportunity to do so until the next open enrollment period, unless you have a Qualifying Life Event.​

Eligible dependents include:

Your legal spouse.

PLEASE NOTE: If your spouse is employed and offered medical coverage by their employer, they are not eligible for coverage under the Jay Group plan. Spousal eligibility form is also required for participation.​

Your children up to age 26. Only those dependents who meet the eligibility requirements can enroll for coverage.
